How the program works:
- Complete the registration form below.
- Your friend will receive an email or a phone call from us to notify them of this opportunity. (You can tell us if you would like to remain anonymous and we will not release your name.)
- Once your friend makes a purchase of at least one hearing aid, you will receive a $50 Visa Gift Card and they will receive a $50 discount.
The more you refer, the more you earn! You may also pick up a Refer-A-Friend card at any Hearing Healthcare Center office.

To qualify, you must have completed the above form and the friend must make a qualifying purchase. All registrations must match the name and address provided to Hearing Healthcare Center upon their first appointment. Returned/invalid information will not be eligible. The $50 gift card and $50 discount will be issued just once regardless of the number of purchases made thereafter.
